Immunization Requirement

Immunization Deadline: Friday, July 25

This is a reminder that you must submit your immunization records to Student Health before you arrive on campus.  Most students have received the required immunizations in the past, and can obtain immunization records from their medical provider, high school, or prior university. 

Please visit our website for additional information including immunization forms and instructions on uploading forms.

You should have received an activation code email from MyChart a few days ago. Please check your junk mail in case it went there. If you did not receive an email or if you need further assistance with MyUHealthChart, please call the MyChart support line at 877-448-1773.

Minor Consent

In addition, if you will be less than 18 years of age when you arrive on campus, please complete the minor consent form and upload to your MyChart account.

Student Health Insurance Plan

UnitedHealthcare Student Resources “UHCSR” is the new student health insurance plan for the 2025-2026 academic year. The new student insurance plan offers comprehensive coverage, competitive pricing, and a robust provider network. More information regarding the new plan is available on the Student Health website.

Student Health Insurance Waiver

Waiver Deadline: Tuesday, July 15

Students who would like to waive the Student Health Insurance charge for 2025-2026 can submit a waiver on the UnitedHealthcare Student Resources waiver portal.

A verification code will be sent to your UM email address, which will be needed to access the waiver form after entering your Student ID (starting with a 5) and Date of Birth. Students will receive an approval or denial within five (5) business days from UHCSR after submitting their waiver.

Please call UnitedHealthcare Student Resources customer service at 1-833-931-0533 should you require assistance with your waiver. Students will receive confirmation that their waiver was approved or denied within 5 business days after submitting their waiver.

Students with approved waivers will have their insurance charge waived from their account before the first billing statement is generated on Monday, July 14. Please remember that waivers must be renewed each fall semester and prior waiver approvals do not carry over to the new academic year. No waivers will be accepted after the July 15 deadline. 

All new students (including transfer students) with Out-of-State HMO/EPO plans do not meet the criteria for waiver unless the policy provides comprehensive health benefits beyond emergency and urgent care during the time that you will attend the University of Miami. Please note that Florida Medicaid is the only Medicaid plan that is eligible for waiver.  Please contact studenthealth@miami.edu if you are interested in applying for Florida Medicaid for further assistance.
