UM Researchers Help Answer Questions About Treatment for Sickle Cell Anemia

Children with sickle cell disease are at risk for suffering a stroke, especially if a transcranial Doppler ultrasound examination, or TCD, has shown a narrowing of the vessels in their head. Following a major study called STOP, the Stroke Prevention Trial in Sickle Cell Anemia, the standard of care for these at-risk youngsters with abnormal TCDs became regular blood transfusions. But a major clinical question remained: How long should these patients continue to receive blood transfusions? Researchers from the University of Miami Leonard M. Miller School of Medicine and other centers across the nation conducted a second trial called STOP 2, and have now provided a dramatic answer to that question, the findings of which were recently published in The New England Journal of Medicine.

“From our study it appears that not only does transfusion prevent stroke, but it actually reverses, at least temporarily, the narrowing of the vessels in the brain,” said Ofelia Alvarez, M.D., associate professor of clinical pediatrics and principal investigator in the STOP 2 trial.

The next area of study is how to prevent a second stroke in children with sickle cell anemia while reducing iron overload from the transfusions. In April, the National Institutes of Health will begin an $18 million five-year study called Stroke with Transfusions Changing to Hydroxyurea (SWiTCH). The Department of Pediatrics will be one of 20 major pediatric sickle cell centers participating in this study.

Learn How to Talk to Your Teens

S.T.E.P. (Systematic Training for Effective Parenting) Teen is an eight-week, midday discussion group conducted by the Employee Assistance Program. This course offers parents a realistic and practical approach to meeting the challenges of raising adolescents today. It helps parents learn effective and enjoyable ways to communicate with their teens and to achieve the goal of raising responsible children. Miami Lite

The Miami Lite program helps participants with weight loss, nutrition, controlling portion sizes, weight management techniques and guidelines for exercise. It is open to the public and is offered every Thursday from 8:30 to 9:30 a.m. at the Diabetes Research Institute, first-floor Education Center. The cost is $20 per class or $150 for ten sessions. Families First Network

The University of Miami’s Families First Network, funded by the Children’s Trust, is offering free parenting groups for parents of newborns and children through age 5. Performance Improvement Management Training

Performance Improvement Management is a highly interactive series of four half-day training sessions and one two-hour follow-up session. The goal of the series is to set expectations for University of Miami leaders and develop the knowledge and skills required to effectively manage employee performance. The course includes three modules: role of the manager, performance improvement management process and documentation. After the sessions, participants should be able to conduct an effective performance conference, explore the impact of non-performance improvement management, assess and analyze performance gaps and develop appropriate interventions, develop and implement action plans to ensure performance change, and document performance issues appropriately.

All classes take place in Dominion Parking Garage, room 155. Morning sessions are from 8:30 a.m. to 12:30 p.m. Afternoon sessions are from 1 to 5 p.m. You must register for all five parts of the session you select. Seating is limited. Miller School of Medicine’s Staff Performance Evaluation Process

This year’s performance evaluations are to be forwarded to departmental human resources representatives and entered into DHRS by May 1. For Bascom Palmer Eye Institute employees, medical human resources will enter the overall evaluation rating for evaluations received prior to May 1. For UMHC employees, evaluation ratings will be transferred electronically from Halogen to DHRS. Staff that do not have performance evaluations entered in DHRS by May 1 will not receive a merit increase in their June paychecks. Delayed merit increases will be retroactive to the effective date of the new rate.

Because of the evaluation and merit process, only transfers and new hires will be processed between May 1 and June 24. Other salary adjustments, reclassifications and/or promotions will be processed prior to May 1 or after June 24, and any such changes will be retroactive to the actual effective date.

Child Car Seat Installation Check

Understanding Your Credit Report

Publications

Rafael E. Campo, M.D.,  associate professor of clinical medicine and a member of the Division of Infectious Diseases, was one of the authors of a study published in the January 19 issue of The New England Journal of Medicine that compared antiretroviral regimens for HIV infection.  The study found that the combination of tenofovir DF and emtricitabine plus efavirenz suppressed viral loads more effectively than the combination of zidovudine and lamivudine plus efavirenz.  The latter has been considered the “gold standard” of antiretroviral therapy over the last few years.  The difference in outcome was attributed to the new treatment having fewer side effects than the older one.

 The AIDS Clinical Research Unit, directed by Margaret Fischl, M.D., is recruiting subjects for an investigational smallpox vaccine in patients with HIV and without HIV. The vaccine uses a modified vaccinia virus, which is not as strong as the smallpox virus and cannot grow well enough in human cells to cause disease. Participants must be 18 to 34 years of age, have never received smallpox vaccinations, and have no history of heart or skin problems. HIV patients must be taking antiretroviral therapy and have a viral load of less than 50,000 and a CD4 cell count greater than 200. Up to $275 provided and free HIV testing is done.  The AIDS Clinical Research Unit is also recruiting subjects for an investigational CC-chemokine receptor 5 (CCR5) entry inhibitor for patients with HIV. Study will look at single dose, dose escalations of a study drug (0.5 mg, 2.0 mg and 5.0 mg) and a placebo. The study will determine how well the study drug blocks CCR5 receptors and is tolerated. Participants must be 18 and older, not taking antiretroviral therapy for at least three months and have a viral load greater than 5,000 and a CD4 count greater than 250. The AIDS Clinical Research Unit, under the direction of Margaret Fischl, M.D., is recruiting participants for a therapeutic HIV-1 vaccine study. Participants must be 18 to 50 years of age, receiving antiretroviral therapy, have an HIV-1 RNA level less than 500 copies/ml for at least two years and a CD4 cell count 500/ccm or higher. The AIDS Clinical Research Unit is also recruiting participants for a phase I IL-7 study (IL-7 is a cytokine that aids T-cell development and improves T-cell survival). Participants must be 18 to 50 years of age, receiving antiretroviral therapy for at least 12 months, have an HIV-1 RNA level less than or equal to 50,000 copies/ml, and have a CD4 cell count of 100 cells/ccm or higher. Participants must be willing to do 12-hour IL-7 blood level monitoring. Up to $200 in food certificates provided. Anique Bryan, M.D., and the clinical faculty of the Department of Family Medicine and Community Health are recruiting patients with type 2 diabetes for a one-year study involving inhaled insulin. This study will focus on patients with uncontrolled diabetes despite being treated with two oral medications for at least two months. Study subjects must be over the age of 35. The SPARE Study (Soy Phytoestrogens As Replacement Estrogen) is currently recruiting women 45-60 years of age to study the role of soy phytoestrogens given in tablet form to prevent bone loss. Participants will be given a physical exam at study entry, an annual mammogram and bone density test, and will be asked to come for 10 study visits over a period of two years for blood tests and to complete questionnaires about diet, exercise, and well-being. The Division of Cosmetic Dermatology, directed by Leslie Baumann, M.D., is looking for people over the age of 18 who have never been treated with botulinum toxin to participate in a research study to determine whether injections with an investigational drug (Botulinum toxin Type A) will correct frown line wrinkles in the glabellar area (between the eyebrows). The Division of Cosmetic Dermatology is also looking for teenagers between the ages of 12 and 17 to participate in a research study to determine whether injections with Botulinum toxin Type A relieve excessive underarm sweating. Carlos Hernandez-Cassis, M.D., is currently recruiting patients ages 18 through 80 with type 2 diabetes for participation in a research study to test the safety and effectiveness of an investigational medication. Bruce Rubin, M.D., is currently performing a study on trigeminal neuralgia (TN) treatment with subcutaneous injections of BOTOX® to evaluate the effectiveness of treating facial pain associated with TN. Dr. Rubin is also seeking patients for a research study on the treatment of tightness/spasticity in the wrist, fingers, and elbow caused by a stroke while evaluating the effect of BOTOX® on pulmonary functioning. Patients must be 18 years of age, have had a stroke six months ago or longer, have a pulmonary/breathing problem and cannot have used BOTOX® or any other botulinum toxin previously for any condition.
